Community Health Needs Assessment
The Affordable Care Act requires hospitals to conduct a Community Health Needs Assessment (CHNA) every three years as a way to evaluate the overall health of the community. The assessment identifies recurring causes of poor health, then focuses resources to support and drive positive change in the identified concerns and behaviors.
By surveying community residents, we learn about the health status of the population, identify areas for health improvement, determine factors that contribute to health issues and identify assets and resources that can be mobilized to address public health improvement.
